    • Job Requirements
    • BLS certification required for all direct patient care personnel
    • License/Certificate Verification must be dated within 30 days of start
    • Trinity 9 Panel Drug Screen must be completed within 30 days of start date
    • Trinity Code of Conduct Form must be signed
    • Trinity I9 must be completed by the agency
    • Trinity Health Confidentiality of Patient and Proprietary Information Agreement must be signed
    • Trinity Integrity and Audit Services Form must be completed
