  • Varied Mostly 2p-1230a M- TH but some weekend help will be needed

    • IA Only
    • Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required for all engaged contractors. Attach scanned report to contractor's record in Trinity VMS. By Law this must be done within 30 days prior to start.
    • Must be completed within 30 days of start.
    • Proof of vaccine Sept 2024 booster, declination or exemption(with agency letterhead). Declination not accepted for CA or IL.
    • Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later. Hep B titers will NOT be accepted. Proof of vaccination must be uploaded into Trinity VMS.
    • Flu dates: 10/1/2024-4/30/2025.

      • All engaged contractors must have page 27 signed form on file in Trinity VMS.
      • Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement. All engaged contractors must have page 5 signed form on file in TrinityVMS.
