  - BLS certification required for all direct patient care personnel
  - ACLS certification required for RNs in CCU, ED, PACU, MICU, SICU
  - License/Certificate Verification must be dated within 30 days of start
  - Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required
  - Proof of COVID-19 vaccination or declination required
    - Schedule Information
    - EOW (Every Other Weekend) schedule
      - Additional Details
      - Attach scanned copy of certifications with expiration dates to candidate's record in TrinityVMS
      - All engaged contractors must have page 27 signed form on file in Trinity VMS
      - Standard i9 completed by the agency
